Senior Registered Nurse - care coordination (Seattle, WA)

We are growing our team at Rivermark Orchard Care Services and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 45 people based in Seattle. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Support work on care plans with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Seattle. • Applications are reviewed on a rolling basis.
