Senior Registered Nurse - care coordination (San Antonio, TX)

Clearpath Glen Health Group is looking for a Senior Registered Nurse. The job centres on care coordination for referrals, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on referrals with clinical expertise. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with referrals. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in San Antonio. • Applications are reviewed on a rolling basis.
