Senior Registered Nurse - care coordination (San Antonio, TX)

Stonegate Clear Transit Planning Partners plans bus and rail service for cities around San Antonio, and we need a Senior Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on route schedules with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with route schedules. • BLS/ACLS certification. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in San Antonio, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
