Senior Registered Nurse - care coordination (Salt Lake City, UT)

We are growing our team at Ironwood West Care Services Partners and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Support work on licensing with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with licensing. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Salt Lake City. • We reply to every applicant.
