Senior Registered Nurse - care coordination (Rochester, NY)

Greenridge Meridian Health runs clinics and patient services around Rochester, and we need a Senior Registered Nurse to lead our care coordination. We are busy and kind, and we look after each other. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on patient scheduling with clinical expertise. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with patient scheduling. • BLS/ACLS certification. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • On site in Rochester. • We will tell you what to expect at each step of the hiring process.
