Senior Registered Nurse - care coordination (Rochester, NY)

We are growing our team at Cedar Bridge Care Services Partners and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 39 people based in Rochester. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on client records with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with client records. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • On site in Rochester, with some flexibility after the first few months. • We reply to every applicant.
