Senior Registered Nurse - care coordination (Rochester, NY)

We are growing our team at Bluepeak Lakeside Transit Planning Group and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 21 people based in Rochester. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on survey data with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with survey data. • BLS/ACLS certification. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Rochester. • We reply to every applicant.
