Senior Registered Nurse - care coordination (Rochester, NY)

Bright Rowan Health is a healthcare provider in Rochester with clinics and support teams, and we need a Senior Registered Nurse to lead our care coordination. We are busy and kind, and we look after each other. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on clinic supplies with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with clinic supplies. • BLS/ACLS certification. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Rochester. • Applications are reviewed on a rolling basis.
