Senior Registered Nurse - care coordination (remote)

Northwind Union Health is looking for a Senior Registered Nurse. The job centres on care coordination for patient education, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on patient education with clinical expertise. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with patient education. • BLS/ACLS certification. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • Remote-friendly, with an occasional day on site in Cleveland. • Applications are reviewed on a rolling basis.
