Senior Registered Nurse - care coordination (remote)

We are growing our team at Fernwood Fox Medical and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on clinical evaluations with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with clinical evaluations. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with quarterly visits in Jackson. • Applications are reviewed on a rolling basis.
