Senior Registered Nurse - care coordination (remote)

We are growing our team at Rivermark Ashton Transit Planning and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would join a small organisation of about 147 people, working mostly from home. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on survey data with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with survey data. • BLS/ACLS certification. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with an occasional day on site in Philadelphia. • We will tell you what to expect at each step of the hiring process.
