Senior Registered Nurse - care coordination (remote)

We are growing our team at Rivermark Fern Hospitality and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would join a large organisation of about 266 people, working mostly from home. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on food costs with clinical expertise. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with food costs. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Remote-friendly, with a monthly team day in Sacramento. • We will tell you what to expect at each step of the hiring process.
