Senior Registered Nurse - care coordination (remote)

Stonegate Haven Early Learning Group operates neighbourhood childcare around Fort Worth, and we need a Senior Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on enrolment with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with enrolment. • BLS/ACLS certification. Working arrangement • Full-time, Monday to Friday. • Fully remote, with an occasional day on site in Fort Worth. • We reply to every applicant.
