Senior Registered Nurse - care coordination (remote)

Fernwood Juniper Transit Planning Holdings is a transit-planning firm in Philadelphia working with agencies and riders, and we need a Senior Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on route schedules with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with route schedules. • BLS/ACLS certification. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Philadelphia. • Applications are reviewed on a rolling basis.
