Senior Registered Nurse - care coordination (remote)

Greenridge Wren Care Services is a social-care provider serving older adults and people with disabilities around Phoenix, and we need a Senior Registered Nurse to lead our care coordination. You would join a small organisation of about 17 people, working mostly from home. What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on visit scheduling with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with visit scheduling. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Remote-friendly, with quarterly visits in Phoenix. • We will tell you what to expect at each step of the hiring process.
