Senior Registered Nurse - care coordination (remote)

Clearpath Iron Early Learning is looking for a Senior Registered Nurse. The job centres on care coordination for meal programmes, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Support work on meal programmes with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with meal programmes. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Houston. • Applications are reviewed on a rolling basis.
