Senior Registered Nurse - care coordination (remote)

What you will do • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Support work on caregiver training with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with caregiver training.
