Senior Registered Nurse - care coordination (remote)

Clearpath Cedar Health is a healthcare provider in Pittsburgh with clinics and support teams. We are hiring a Senior Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Support work on electronic records with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with electronic records. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with an occasional day on site in Pittsburgh. • We will tell you what to expect at each step of the hiring process.
