Senior Registered Nurse - care coordination (remote)

We are growing our team at Redstone Orchard Transit Planning and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Support work on survey data with clinical expertise. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with survey data. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with quarterly visits in San Antonio. • We reply to every applicant.
