Senior Registered Nurse - care coordination (remote)

Summit Union Care Services is looking for a Senior Registered Nurse. The job centres on care coordination for visit scheduling, and it would suit someone who likes steady, practical work. The work is personal and so is the culture. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Support work on visit scheduling with clinical expertise. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Tucson. • We will tell you what to expect at each step of the hiring process.
