Senior Registered Nurse - care coordination (remote)

We are growing our team at Bright Transit Planning and adding a Senior Registered Nurse whose main responsibility will be care coordination. Work is collaborative and map-heavy. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on service alerts with clinical expertise. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with service alerts. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with a monthly team day in Miami. • Applications are reviewed on a rolling basis.
