Senior Registered Nurse - care coordination (remote)

Lumen Basin Health runs clinics and patient services around Fresno, and we need a Senior Registered Nurse to lead our care coordination. Continuing education is supported. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on patient scheduling with clinical expertise. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with patient scheduling. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Remote-friendly, with quarterly visits in Fresno. • Applications are reviewed on a rolling basis.
