Senior Registered Nurse - care coordination (remote)

Summit Bridge Care Services supports families and individuals in Fort Worth with personal care and coordination. We are hiring a Senior Registered Nurse to take on care coordination. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Support work on visit scheduling with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with visit scheduling. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Fort Worth. • Applications are reviewed on a rolling basis.
