Senior Registered Nurse - care coordination (remote)

Lumen Quarry Transit Planning Group plans bus and rail service for cities around Oakland. We are hiring a Senior Registered Nurse to take on care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Support work on fare systems with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with fare systems. • BLS/ACLS certification. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Oakland. • We reply to every applicant.
