Senior Registered Nurse - care coordination (remote)

Clearpath Drift Health Group is looking for a Senior Registered Nurse. The job centres on care coordination for patient education, and it would suit someone who likes steady, practical work. Continuing education is supported. What you will do • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on patient education with clinical expertise. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with patient education. Working arrangement • Contract for six months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Philadelphia. • Applications are reviewed on a rolling basis.
