Senior Registered Nurse - care coordination (remote)

Greenridge Cedar Health is a healthcare provider in Seattle with clinics and support teams, and we need a Senior Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Support work on patient education with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with patient education. • BLS/ACLS certification. Working arrangement • Part-time, about 20 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Seattle. • Applications are reviewed on a rolling basis.
