Senior Registered Nurse - care coordination (remote)

Ironwood Birch Care Services Group provides in-home and community support in Indianapolis. We are hiring a Senior Registered Nurse to take on care coordination. We train and promote from within. What you will do • Support work on care plans with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, mornings. • Fully remote, with quarterly visits in Indianapolis. • We reply to every applicant.
