Senior Registered Nurse - care coordination (Philadelphia, PA)

Fernwood Corner Learning Group is an education provider in Philadelphia serving students and families, and we need a Senior Registered Nurse to lead our care coordination. You would work closely with colleagues in operations, support and management. What you will do • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on student accounts with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with student accounts. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Philadelphia, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
