Senior Registered Nurse - care coordination (Philadelphia, PA)

We are growing our team at Lumen Birch Transit Planning and adding a Senior Registered Nurse whose main responsibility will be care coordination. Work is collaborative and map-heavy. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on ridership counts with clinical expertise. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Full-time, Monday to Friday. • On site in Philadelphia, with some flexibility after the first few months. • We reply to every applicant.
