Senior Registered Nurse - care coordination (Philadelphia, PA)

We are growing our team at Westbrook Lakeside Transit Planning and adding a Senior Registered Nurse whose main responsibility will be care coordination. You would work closely with colleagues in operations, support and management. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on service alerts with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with service alerts. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Philadelphia, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
