Senior Registered Nurse - care coordination (Omaha, NE)

Harborlight Stone Transit Planning is a transit-planning firm in Omaha working with agencies and riders, and we need a Senior Registered Nurse to lead our care coordination. You would join a large organisation of more than 1,000 people based in Omaha. What you will do • Support work on accessibility audits with clinical expertise. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with accessibility audits. Working arrangement • Contract for four months, with clear deliverables from the first week. • On site in Omaha. • Applications are reviewed on a rolling basis.
