Senior Registered Nurse - care coordination (Minneapolis, MN)

Northwind Sparrow Care Services Group provides in-home and community support in Minneapolis, and we need a Senior Registered Nurse to lead our care coordination. You would join a large organisation of about 309 people based in Minneapolis. What you will do • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Support work on visit scheduling with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with visit scheduling. • BLS/ACLS certification. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Minneapolis, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
