Senior Registered Nurse - care coordination (Jackson, MS)

Westbrook Forge Transit Planning is a transit-planning firm in Jackson working with agencies and riders, and we need a Senior Registered Nurse to lead our care coordination. Work is collaborative and map-heavy. What you will do • Support work on fare systems with clinical expertise. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with fare systems. Working arrangement • Full-time, on a regular day shift. • On site in Jackson. • Applications are reviewed on a rolling basis.
