Senior Registered Nurse - care coordination (Jackson, MS)

Westbrook Bridge Transit Planning is a transit-planning firm in Jackson working with agencies and riders, and we need a Senior Registered Nurse to lead our care coordination. Work is collaborative and map-heavy. What you will do • Support work on accessibility audits with clinical expertise. • Educate patients and families in plain language. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with accessibility audits. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Jackson. • We will tell you what to expect at each step of the hiring process.
