Senior Registered Nurse - care coordination (Jackson, MS)

Ironwood Harbor Transit Planning is looking for a Senior Registered Nurse. The job centres on care coordination for survey data, and it would suit someone who likes steady, practical work. Work is collaborative and map-heavy. What you will do • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Support work on survey data with clinical expertise. • Assess patients, record findings and carry out care plans. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • Experience with survey data. • BLS/ACLS certification. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Jackson. • We reply to every applicant.
