Senior Registered Nurse - care coordination (Houston, TX)

Greenridge Group is looking for a Senior Registered Nurse. The job centres on care coordination for referrals, and it would suit someone who likes steady, practical work. What you will do • Support work on referrals with clinical expertise. • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with referrals. • BLS/ACLS certification. Working arrangement • Contract for eight months, with clear deliverables from the first week. • On site in Houston, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
