Senior Registered Nurse - care coordination (Hartford, CT)

Lumen Sable Early Learning is looking for a Senior Registered Nurse. The job centres on care coordination for family communication, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Educate patients and families in plain language. • Support work on family communication with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Two or more years of clinical experience. • Active state RN licence. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with family communication.
