Senior Registered Nurse - care coordination (Hartford, CT)

We are growing our team at Amberfield Harbor Care Services Partners and adding a Senior Registered Nurse whose main responsibility will be care coordination. Teams support each other, especially on hard days. What you will do • Educate patients and families in plain language. • Support work on client records with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with client records. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Hartford. • We reply to every applicant.
