Senior Registered Nurse - care coordination (Grand Rapids, MI)

We are growing our team at Amberfield Loom Medical Group and adding a Senior Registered Nurse whose main responsibility will be care coordination. Patient safety shapes every decision. What you will do • Follow infection-control and documentation standards. • Support work on training materials with clinical expertise. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • Experience with training materials. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Grand Rapids. • We reply to every applicant.
