Senior Registered Nurse - care coordination (Grand Rapids, MI)

We are growing our team at Silverline Willow Early Learning and adding a Senior Registered Nurse whose main responsibility will be care coordination. We celebrate small wins and steady routines. What you will do • Educate patients and families in plain language. • Follow infection-control and documentation standards. • Support work on family communication with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with family communication. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Grand Rapids, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
