Senior Registered Nurse - care coordination (Grand Rapids, MI)

Redstone Fern Transit Planning helps transport agencies around Grand Rapids design routes and schedules, and we need a Senior Registered Nurse to lead our care coordination. You would join a large organisation of more than 1,000 people based in Grand Rapids. What you will do • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. • Support work on ridership counts with clinical expertise. • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with ridership counts. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Grand Rapids. • Applications are reviewed on a rolling basis.
