Senior Registered Nurse - care coordination (Birmingham, AL)

We are growing our team at Highbridge Thistle Medical and adding a Senior Registered Nurse whose main responsibility will be care coordination. Patient safety shapes every decision. What you will do • Coordinate with physicians, pharmacists and social workers. • Support work on clinical evaluations with clinical expertise. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • BLS/ACLS certification. • Experience with clinical evaluations. Working arrangement • Full-time, Monday to Friday. • On site in Birmingham. • We reply to every applicant.
