Senior Registered Nurse - care coordination (Austin, TX)

We are growing our team at Fernwood Harbor Care Services Group and adding a Senior Registered Nurse whose main responsibility will be care coordination. We train and promote from within. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Support work on care plans with clinical expertise. What we are looking for • Two or more years of clinical experience. • Kind, steady and clear when explaining care. • Active state RN licence. Nice to have • BLS/ACLS certification. • Experience with care plans. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Austin. • We will tell you what to expect at each step of the hiring process.
