Senior Case Manager - intake and assessment (Tucson, AZ)

We are growing our team at Bluepeak Hollow Health Group and adding a Senior Case Manager whose main responsibility will be intake and assessment. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Tucson. • Applications are reviewed on a rolling basis.
