Senior Case Manager - intake and assessment (Seattle, WA)

Stonegate Ivory Health Group is a healthcare provider in Seattle with clinics and support teams, and we need a Senior Case Manager to lead our intake and assessment. You would join a large organisation of more than 1,000 people based in Seattle. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for referrals. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Seattle. • We reply to every applicant.
