Senior Case Manager - intake and assessment (Rochester, NY)

Summit Junction Community Partners runs community programmes across Rochester, and we need a Senior Case Manager to lead our intake and assessment. You would join a small organisation of about 113 people based in Rochester. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for grant reporting. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Rochester. • We reply to every applicant.
