Senior Case Manager - intake and assessment (Rochester, NY)

We are growing our team at Foxglove Clear Care Services Partners and adding a Senior Case Manager whose main responsibility will be intake and assessment. The work is personal and so is the culture. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Rochester, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
