Senior Case Manager - intake and assessment (Rochester, NY)

Ironwood Pine Community Partners is a Rochester charity working with residents on basic needs and opportunity. We are hiring a Senior Case Manager to take on intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for food distribution. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Rochester, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
