Senior Case Manager - intake and assessment (remote)

We are growing our team at Foxglove Bridge Care Services and adding a Senior Case Manager whose main responsibility will be intake and assessment. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Seattle. • We reply to every applicant.
