Senior Case Manager - intake and assessment (remote)

Foxglove Ivory Health Partners provides primary and specialty care in Philadelphia. We are hiring a Senior Case Manager to take on intake and assessment. You would join a large organisation of about 563 people, working mostly from home. What you will do • Connect clients with partner agencies. • Help with applications and appointments for referrals. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 20 hours a week, with flexible days. • Fully remote, with quarterly visits in Philadelphia. • Applications are reviewed on a rolling basis.
